Terms & Conditions
1. Services Provided
Preferred Choice Home Care offers non-medical home care services, including but not limited to personal care, companionship, light housekeeping, meal preparation, and assistance with daily living activities. Specific services will be outlined in a written Service Agreement tailored to each client’s needs.
2. Eligibility
Services are available to individuals who:

Have completed an intake and assessment.

Have agreed to the Service Plan and applicable fees.

Are legally authorized to enter into this agreement (or have a legal guardian/representative do so on their behalf).
3. Fees & Payments

Fees for services will be provided in writing before services begin.

Payment is due according to the agreed-upon schedule (weekly, bi-weekly, or monthly).

Late payments may result in suspension of services until the account is current.

Any additional costs (e.g., supplies, mileage reimbursement, or special requests) will be discussed and billed separately.
4. Cancellations & Scheduling

Clients must provide at least 48 hours’ notice for cancellations.

Missed or late cancellations may be subject to a cancellation fee.

We reserve the right to reschedule services in the event of emergencies, caregiver illness, or unsafe conditions.
5. Client Responsibilities
Clients (or their representatives) agree to:

Provide a safe home environment free of hazards.
-
Treat caregivers with respect and dignity.
-
Notify us promptly of changes in health, service needs, or emergency contacts.
6. Caregiver Conduct
Our caregivers are thoroughly screened and trained. They are not permitted to:

Handle client finances, including banking, credit cards, or cash.

Provide medical services beyond their scope (unless otherwise licensed).

Transport clients without prior approval from the company.
7. Liability
While we strive to provide safe, professional, and reliable care, Preferred Choice Home Care is not liable for:

Medical outcomes, unless otherwise agreed upon in writing.

Injuries resulting from client non-compliance, unsafe home environments, or refusal of recommended services.

Loss or damage to personal property unless caused by proven caregiver misconduct.
8. Termination of Services
Either party may terminate services with 30 days’ written notice. We reserve the right to terminate immediately if:

Payments are not made as agreed.

Caregivers are subjected to harassment, abuse, or unsafe conditions.

Client needs exceed the scope of our services.
9. Confidentiality & Privacy
We are committed to protecting client confidentiality. All personal, health, and financial information will be handled in compliance with HIPAA and applicable privacy laws.
10. Changes to Terms
We may update these Terms & Conditions at any time. Updated terms will be posted on our website and available upon request. Continued use of our services constitutes acceptance of the updated terms.
11. Governing Law
These Terms & Conditions shall be governed by the laws of the state of Indiana.

1. Payment Methods

Accepted forms of payment (credit/debit card, ACH transfer, insurance where applicable, etc.)

Third-party processors (like Stripe, PayPal, etc., if used) will be handled transactions securely.
2. Modifications to Services/Offerings

We reserve the right to update, modify, or discontinue services or website content without prior notice.
3. No Warranties Disclaimer

Services are provided “as is”, and while we strive for accuracy, we cannot guarantee uninterrupted access, error-free content, or outcomes beyond your scope of care.
4. Intellectual Property & Copyright

Our company owns the website content (text, images, logo, etc.) and it cannot be copied, distributed, or used without permission.
5. Limitation of Liability

We are not liable for damages arising from website use, service reliance, or third-party links.
6. Privacy & Data Protection
